Population Health Fellow
Who we are
The Health Plan of San Mateo (HPSM) is a local non-profit health plan that offers health coverage to San Mateo County's underserved population. We currently serve more than 140,000 county residents through several programs, with most of our members qualifying for coverage through Medi-Cal, the state’s Medicaid program. HPSM has more than 275 employees and partners closely with clinicians and community organizations throughout the region.
Objective
The HPSM Fellowship will provide an enriched educational and professional experience for those who are pursuing an education and career in the field of health care.
Fellowship Scope
This opportunity will provide the fellow with:
· Knowledge and experience in the overall operation of a non-profit community-based health plan.
· Knowledge and experience in the development of programs and services that benefit our diverse membership.
· Professional experience working on a specific project important to HPSM.
Specific Projects Include:
§ Deep dive needs assessment of Seniors & Persons with Disabilities (SPD) population
§ Inventory of current programs and services and subsequent gap analysis
§ Specific Assessment of IHSS and caregiving – gaps in caregiving for members across ILOS
§ Older adults and mental health resources/toolkit for members/caregivers
§ Health education/preventive health education content
§ PCP/network guidance toolkit for serving older adults and persons with disabilities
Fellowship Duration
· Start date: Jan 10, 2022 (flexible based on the needs of the student and HPSM sponsor)
· Duration: 20 weeks minimum with potential for additional work based on project scope
· Hours: Flexible (work schedule to be coordinated with the Fellowship sponsor)
Required Skills & Knowledge
· Currently enrolled in an undergraduate or graduate degree program in public health, public policy, business administration, social work, social sciences, or related disciplines. Minimum 3.0 GPA.
· Basic understanding of the fundamentals of health care and the health care system.
· Solid written and verbal communication skills.
· Self-starter with the ability to work independently as well as work well with others.
· Proficiency with Microsoft Office including Word, Excel, Outlook and PowerPoint.
